01Panoramic x-ray & planning
A panoramic x-ray (or CBCT for complex cases) shows the position of each wisdom tooth, root angles, and proximity to the nerve. We plan accordingly.
02Sedation & anesthesia
Local anesthetic always - plus optional nitrous oxide (laughing gas) or oral sedation if you'd like to be relaxed during the procedure.
03Extraction
15-30 minutes per tooth for straightforward cases. For impacted teeth, a small incision and sometimes minor bone removal - still usually under an hour total.

01Day of surgery
Keep gauze on extraction sites for 30-45 minutes. Apply ice packs — 20 minutes on, 20 off. Stick to cool, soft foods. Do not use a straw or spit forcefully to avoid dry socket.

Days 2 through 3
Swelling peaks around 48 hours then subsides. Continue soft foods and gentle saltwater rinses starting 24 hours after surgery. Take pain medication on schedule.

Days 4 through 7

Information for parents of teens.
Most wisdom tooth patients at our Fort Worth office are between 16 and 22. Dr. Appiah and Dr. Patel use panoramic imaging and CBCT scans to assess exactly how the wisdom teeth are positioned.
We show you the images, explain findings in plain language, and only recommend extraction when clinically necessary. We schedule most teen extractions during school breaks when possible. The procedure typically takes 45-90 minutes for all four teeth.

Do all wisdom teeth need to be removed?
No - if they've fully erupted, fit your jaw, and aren't causing problems, leaving them is fine. We assess at your visit and only recommend removal when there's a clinical reason.
Does it hurt?
During the procedure: no - local anesthetic numbs the area completely. After: typical post-extraction soreness for 3-5 days, managed with over-the-counter or prescribed pain relievers.
How long is recovery?
Most patients are back to normal eating and activities within 5-7 days. Full gum healing takes 3-4 weeks. The first 24-48 hours have the most swelling and discomfort.
When should I get my wisdom teeth out?
Late teens or early 20s is the sweet spot - roots aren't fully formed yet, healing is fastest, and complications are lowest. Older adults can still have them removed safely; recovery just takes a bit longer.
What if my wisdom teeth are deeply impacted?
For complex cases - full bony impaction, proximity to the nerve, or unusual root anatomy - we may refer to an oral surgeon. We'll always be honest about what we can and can't handle in-house.
Can I drive home after the procedure?
With just local anesthetic, yes. With nitrous oxide, you're fine within 5 minutes after. With oral sedation, you'll need someone to drive you home and stay with you for a few hours.
What is dry socket, and how do I prevent it?
Dry socket occurs when the blood clot at the extraction site becomes dislodged, exposing the underlying bone. It causes increased pain around days 3 to 5. To reduce your risk, avoid straws, smoking, and spitting for at least 72 hours after surgery. If you develop dry socket, call us — we can place a medicated dressing that relieves pain quickly.
How much does wisdom tooth removal cost with insurance?
With PPO dental insurance, wisdom tooth extraction is typically covered at 50% to 80% after your deductible. Out-of-pocket costs range from $75 to $200 per tooth depending on impaction level. Without insurance, our self-pay rate is $299 per tooth.

Yes, and in most cases we recommend it. Removing all four teeth in a single visit means one round of anesthesia, one recovery period, and one set of follow-up appointments. The procedure typically takes 45 to 90 minutes depending on impaction level.
